This is a leadership position and requires the skill set for integrative care coordination in the case management department to include direction, oversight, and accountability for the social work team. The position will lead processes for developing the care coordination team collaboration of SW and RN case managers. Responsible for assisting with and maintaining referral resources for post-acute services for patients. Provide direction for discharge planning of patients with a focus on complex/difficult discharge patients in the acute care setting. The position will provide supervision and mentoring for social work licensure and/or certification of subordinates. Provides structured mentoring to develop the appropriate skill set within the social work team members with a concentrated focus on discharge planning. This also includes developing best practices, developing policy and guidelines for social work functions. Displays excellent communication, presentation and group facilitation skills. Responsible for managing issues to the case management manager/director.
Minimum Education Requirements
Master’s degree in Social Work (MSW)

3 years recent experience in social work in a hospital setting.

Assess/reassess a patient’s clinical and psychosocial status, pre-morbid status, community services utilized, and diagnosis and treatment plan per case management referral. Through assessment process identifies community resources needed and facilitates referrals to agencies (local and state) or programs for assistance as needed. Educates family and/or patient on community resources available for assistance. Documents actions taken in progress notes in EMR/and case management software.*
Provides accountability in discharge planning to next level of care.*
Maintains knowledge of regulatory requirements for discharge planning and role of social workers.*
Conducts and monitors informal and formal family conferences along with nursing staff to ensure the most efficient coordination of health care and related services needed.*
May conduct audits regarding documentation and outcomes for case management.*
Act as an expert resource to staff regarding department, system, policy and procedure changes. Acts as a liaison with physician patients, nursing staff as appropriate.*
Consults and communicates, as appropriate, with colleagues and manager/director regarding difficult practice issues. Adheres to state and federal regulations pertaining to discharge.*
