Works with providers and patients to ensure chronic disease metrics are met for each patient and patient population. Reviews
patient interim health encounters and problem solves challenging areas. Verbalizes an understanding of the value of measurement and the importance of continuous quality improvement data. Use quality measures to improve performance and accountability for patient outcomes, patient experiences and safe delivery of care. Assists with coordination of clinic-based Quality improvement, evidence-based practice and research activities. Use current healthcare research findings and other evidence to expand clinical knowledge, enhance role performance, and increase knowledge of professional issues. Develops working relationships with community resources for the benefit of patients. Participates and coordinates community outreach efforts on behalf of the clinic to improve community health.
PATIENT & FAMILY CENTERED CARE - 20%
Promote patient education, self-management support and timely delivery of health care. Identify and work with individual and
patient populations as required for clinical or medical reasons.Responsible for participation with teams in strategizing patient
care, then operationalizing the strategy. Responsible for maintaining protocols and methods to achieve patient standards
related to quality care, customer satisfaction and improved patient outcomes. Identifies patients not meeting clinical goals and
creates patient, physician, provider and clinic level quality performance reports. Identifies patients overdue for visits, labs, or
referrals and initiates patient supportive services to reduce negative impact on patients' health, and promote efficacy of clinic
resources. Provides pre-visit planning workflow to ensure care completion prior to visit whenever possible. Responsible for
promoting coordinated services and transitions of care within the clinic and with outside health providers. Guides the patient, using
motivational interviewing skills, addressing critical issues and selfmanagement goals and tasks. Tailors the content from patient
education materials to the needs and priorities of the patient during each intervention contact. Acts a liaison with clinic providers, specialty providers, office staff and patients to develop improved integrated processes and patient outcomes.
INFORMATICS/TECHNOLOGY - 20%
Adapts to rapidly changing technology as evidenced by competency in technology and equipment used in the patient care
setting. Thorough and confidential maintenance of patient charts and records. Document ongoing status, interventions, patient
response and outcomes in accurate, timely manner. Use information technology to communicate, manage knowledge,
mitigate error, and support decision-making. Maximizes the use of the registry function to assure processes in place to collect, analyze and report select data to ensure effectiveness of patient plan of care and Patient Centered Medical Home (PCMH) model in clinic population management.
COORDINATION OF PATIENT CARE - 10%
Maintains records, reports, logs, and files in a HIPPA compliant fashion as required. Collects, tracks, and reports data as required.
Assists and coordinates various functionality and utilization of disease registry database. Provides and receives immediate
feedback for all stakeholders for clinic process improvement and patient self-efficacy. Engages clinic and community programs/services in support of patients self-management of chronic conditions. Works with IT resources to facilitate registryreporting at the site and documentation of data in searchable fields. Demonstrate strong project management skills,communications skills, team orientation, respect for others, critical thinking skills. Ability to interpret, adapt and apply guidelines and protocols. Ability to self-motivate, prioritize, and be willing to invest in change processes to improve efficiencies.
PROFESSIONAL DEVELOPMENT - 10%
Seeks out learning opportunities and continuing education to improve care. Seeks experiences that reflect current practice in
order to maintain skills and competence in practice or role performance. Maintains professional records that provide evidence of competency and lifelong learning. Evaluates one's own integrity and practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations.Takes action to achieve goals identified during the evaluation process. Demonstrates a commitment to the organization through ongoing participation in clinic based meetings.
