CLINIC: Centura Health Physician Group (CHPG) Ridgeline Family Medicine - Castle Rock, CO
SCHEDULE: Monday - Friday, business hours
SHIFT: This is a full time position requiring 40 hours per week
Occasional travel to other sites to meet the needs of the clinic may be required.
Responsible for coordinating and providing depression, anxiety, and other behavioral health care for patients in primary care clinics. Educates patients about behavioral health issues and treatment options; provides behavioral activation; monitors behavioral health symptoms and response to medication and/or psychotherapy using structured instruments (e.g., the PHQ-9, GAD-7, or other appropriate instruments, etc.); works closely with the primary care provider and external mental health resources in the community to revise the treatment plan when patients are not improving; and offers a brief course of counseling for depression and other behavioral health issues (e.g., Problem Solving Treatment in Primary Care (PST-PC or other appropriate modality) when indicated.
Minimum Education Requirements
Master's Degree in Social Work or Masters in Counseling
Minimum Experience Requirements
Prior formal training and experience in brief, structured counseling techniques (e.g. cognitive behavioral therapy (CBT) or Problem Solving Treatment (PST), etc.).
Competence with DSM diagnostic criteria and treatment planning.
Effective written and verbal communication skills.
Demonstrated ability to establish rapport quickly with a wide range of people.
Demonstrated ability to work within and coordinate a care team comprised of internal team members and community-based members.
Minimum 2 years direct clinical experience delivering behavioral health care in a relevant setting.
Preferred experience: Experience with design, implementation, or maintenance of the IMPACT model; Screening, Brief Intervention and Referral to Treatment (SBIRT) model, or other appropriate evidence-based models of behavioral health care delivered in a primary care setting. Experience working with medically ill and/or older adults in a primary care setting preferred.
State of Colorado Licensed Clinical Social Worker (LSCW) or Licensed Professional Counselor (LPC) or a registered psychotherapist
Current Basic Life Support from the American Heart Association or other certifying agency.
Position Duties (essential functions denoted with an * )
Conducts assessment of patients, including completion of the Patient Health Questionnaire (PHQ-9) depression scale, General Anxiety Disorder-7 (GAD-7) anxiety scale, Alcohol Use Disorders Identification Test (AUDIT), and other standardized behavioral health assessments deemed appropriate by the BH Provider Appropriately assesses suicide risk.*
Conducts initial visit including detailed behavioral health history, assessment, and education about the nature of behavioral health issues and the goals and expectations of treatment.*
Consults with patient and primary care provider about treatment options and preferences; coordinates initiation of treatment plan. Works with interdisciplinary team to formulate and implement care of patient using insight, limit setting, goal setting and other techniques as appropriate. Applies knowledge of psychiatric medications and ability to evaluate responses.*
Monitors patient closely (in-person or by phone) for changes in severity of symptoms and medication side effects; educates patients about medications and medication side effects, as needed; encourages treatment adherence.*
Uses behavioral activation techniques with patients as an adjunct to other treatments.*
Provides evidence-based, brief structured psychotherapy (e.g., PST-PC) or interventions (i.e. Brief Intervention for alcohol abuse) as indicated and accepted by patient.*
Coordinates and facilitates communication between patient, primary care physician and external psychiatrist or other resources; provides recommendations for change in treatment plan and supports implementation of new plan.*
Documents all encounters in the Electronic Medical Record in a timely manner according to organizational policies and procedures; monitors outcome measurements Applies knowledge of state statutes criteria pertaining to involuntary holds, seclusion and restraints.*
Facilitates treatment referrals, as needed.*
