Under general supervision and according to established home care and hospice polices, assists patients, families and caretakers in coping with problems from severe to long-term illness, terminal illness, with difficulties in recovery and rehabilitation. Responsible for assessing the psychosocial needs of patients, families and caretakers and providing intervention to meet those needs utilizing personnel, family and community resources. The incumbent will also provide crisis intervention, grief therapy, case consultation and collaborations to other professional and interdisciplinary team.
Graduate of Accredited Bachelor's in Social Work Program required
Graduate of Accredited Master's in Social Work Program Preferred
work experience requirements
Knowledge of community resources used for discharge planning, hospital operations, excellent communication/presentation skills, knowledge of third party payment systems, Medicare/Medicaid programs.
Maintains current knowledge base of community services through continuing education.
Ability to multi-task, set priorities and maintain organization. Computer skills.
Experience in Social Work with emphasis on discharge planning, referral to community services and/or case management or other related experience.
Current Kansas LBSW License required
Current Kansas LMSW License preferred
