POSITION DUTIES (ESSENTIAL FUNCTIONS, INCLUDE % OF TIME)
MEDICATION ASSESSMENT - 20%
Interprets, evaluates, and monitors patient medication orders in a timely manner using the patient medical record and / or pharmacy profile.
Dispenses medication in an accurate and timely manner following hospital formulary guidelines.
Participates in Code Blue and other emergency situations to ensure adequate drug supply and appropriate drug use.
Works with hospice and other outpatient services to ensure adequate drug supply and appropriate drug use.
Demonstrates high technical skill level in utilizing a variety of pharmaceutical competencies including, but not limited to, calculations, aseptic technique, cytotoxic drug properties, vancomycin / aminoglycosides, infectious disease, TPN, and Pharmacokinetics management.
COLLABORATION - 20%
Works effectively with physicians and other healthcare providers to provide high quality, cost-effective use of pharmaceuticals.
Provides drug information to the health care team by verbal communications, inservices, memos, and newsletters to ensure appropriate prescribing and administration of medications.
Provides drug information to patients by verbal and written communication to ensure understanding of prescribed medications.
Provides age-appropriate patient counseling to foster patient understanding and optimal outcome.
PROBLEM SOLVING - 20%
Recognizes and resolves inappropriate orders for medications using amassed technical knowledge and effective communication with the health care team.
Prioritizes daily workload demands by balancing dispensing requirements with time requirements for the resolution of questionable orders.
Provides work direction to technicians, interns and externs.
Attains professional growth by attending or participating in a minimum of 25 hours of continuing education programs annually.
Performs special projects and other activities as assigned to increase and improve level of service, skill level, and productivity.
Demonstrates ability to remain calm and provide appropriate care in emergency situations.
LEADERSHIP - 20%
Responsible for the implementation and follow through of MUE programs as established by the Clinical Coordinator, thus assuring cost effective drug use in the hospital.
Participates as a member in all interdisciplinary clinically coordinated committees' thus maintaining communication and information lines between the pharmacy and other health care professionals.
Performs in-depth analysis of new medications with high price tags so that cost-effective yet quality drug therapy can be maintained
Assists in design, promotes and actively participates in department-based P.I program.
Professional role model for all staff, demonstrating high technical ethical / legal professional pharmacy practices.
Delegates to non pharmacy personnel in accordance with state Board of Pharmacy and hospital policy.
Conducts individualized orientation to new employees and provides ongoing coaching to all employees, conducts training, provides informal education opportunities.
Identify clinical and operational needs of department and coordinate with peer teams and management to develop and implement interdisciplinary action plan.
Actively supports management in establishing standards of practice, noting non-compliance, and participating in corrective action plans.
DOCUMENTATION - 20%
Maintains patient and other records using pharmacy computer and manual systems to ensure timely and accurate patient billings and compliance with regulatory agencies.
Maintains documentation of clinical activities for salary cost justification and facilitates maintenance of adequate supplies by logging need in Want book.
