Under general supervision, the Coder II is responsible for facilitating the organization's Coding Proficiency Program. Responsibilities include performing concurrent medical chart audits on ICD-10 coding and documentation, interfacing and disseminating audit results to internal team, supporting team on coding-related questions, and assuring that encounters are coded to meet compliance and coding regulations and guidelines. This position is a primary resource for coding compliance. The Coder II actively participates in special project work as needed. The ability to compare coding with applicable dictation is mandatory.
Minimum Education Requirements
High school diploma required.
Degree from a two-year college preferred.
Minimum Experience Requirements
2-3 years medical coding experience required in a physician setting
Medical Chart/charge audit experience desired
Medical terminology/Coding Education experience desired
Basic computer, word processing, excel spreadsheet and 10-key skills required
HCC experience desired
License and Certification
Coding certification through AAPC or AHIMA required
Reviews EMR, PDL forms and applies coding training to identify and address appropriate documentation to support ICD-10 codes.
Accurately enters audit results into information into the designated computer software system.
Performs medical chart audits meeting minimum productivity standards and ensures proper chart documentation.
Proactively disseminates coding findings to teams and practices.
Submits and follows up on query forms to clinicians based on chart audit data.
May have the opportunity to work with clinician departments to review and explain medical chart audit results.
Provides updates and status reports to management on a weekly basis
Submits daily audit totals to team.
Stays abreast of industry coding and compliance issues.
Participates in coding I auditing discussions with team to ensure best practice efforts and processes are implemented ensuring maximum reimbursement through appropriate coding
Maintains coding skill through continuing education and reading of distributed materials. Trains new coders as requested in the coding and audit process.
Demonstrates excellence in communication skills and positive interpersonal relations.
Acts as a service representative of the department as needed.
Meets department and system attendance standards.
Actively seeks to promote and helps to maintain a professional, team oriented, service conscious environments, which contributes to the goals of the department and reflects the values of the system.
Supports chart/claim audit special projects.
Assists with development and/or reviews of coding guidelines, policies, procedures as needed.
Consistently exhibits behavior and communication skills that demonstrate FullWell's commitment to superior customer service, including quality, care and concern with each and every internal and external customer.
