Based on a community health assessment and input from local community leaders and neighbors, Centura Health decided to transform St. Anthony North Hospital to a facility that provides primary care and crisis care, while retaining the most needed outpatient services. Centura Health transitioned St. Anthony North Hospital into 84th Avenue Neighborhood Health Center on March 3, 2015 to meet the surrounding community’s health needs and provide solutions where identified gaps in health care services and resources existed. Many of the outpatient services offered at St. Anthony North Hospital, including an ER, remain and new services have been added, including primary care, crisis care and specialty physician practices, to provide even more care in one convenient location.
