Assists CMS director, as assigned, in implementing strategic direction; managing human, physical and financial resources to deliver optimal health outcomes for patients; and maintaining high level professional and technical skills needed to ensure effective management and leadership.
Assists director in coordinating initiatives focused on building, fully integrating partnerships with Physician Groups, Acute Care, Ambulatory Care and Post-Acute Care entities, internal and external.
MINIMUM EDUCATION REQUIREMENTS
Bachelor's degree in nursing (BSN), social work (BSW, MSW preferred)
RN or LSW
MINIMUM EXPERIENCE REQUIREMENTS
4 years clinical experience as a registered nurse or social worker
3 years progressive experience in case management to include discharge planning and utilization management.
Coordination service delivery—Utilization Management, Discharge Planning
-Supports Director in identifying, developing and implementing programs that enhance optimal patient outcomes.
-Demonstrate high competency in care management by providing services that demonstrate exceptional skill in the care management discipline.
- Oversees day-to-day service delivery in assigned areas of patient care management including being initial point of contact for case consultation in these areas of responsibility.
-Support function for case manager account specialists.
-Serves as key point of contact with internal and external partners engaged along the continuum of care.
-Participate in system, corporate and community committees as assigned.
Human Resources Management
-Manages human resources in assigned areas of responsibility, including ensuring appropriate staff coverage and utilization.
- Participates in recruitment, interviewing and hiring of appropriate staff.
-Provides orientation, training and oversight for staff in assigned areas of responsibility.
-Works with Director and department scheduler on staffing and coverage.
-Assists, as assigned, in completion of PFD process for staff.
-Monitors associate satisfaction on an ongoing basis and work with Director to increase satisfaction

Cost/Quality Improvement—including KPI
-Assist Director in answering compliance with regulatory agencies in the following ways:
*Providing and documenting continuing education for staff.
*Review and revision of policies in line with Center for Medicare and Medicaid Services and Joint Commission
-Analyze outcome and variance data in conjunction with Quality Resources and identify potential practice changes, clinical path development opportunities or initiatives to improve the quality and cost effectiveness of care delivered.
- Assists Director with operations and capital budget preparation by projecting departmental needs.
-Monitor monthly fiscal performance against budget.
- Manages/controls staffing levels in assigned areas of responsibility to ensure productivity standards are met/exceeds.
- Assists Director in setting and being a champion for the department's strategic direction.
-Models ability to change and adapt.
-Models and promotes Penrose-St. Francis mission and values.
-Assures decision making is based on values and promotion of a healthy corporate culture.
-Maintains strong interpersonal relationships with staff as well as all other internal and external partners based on mutual respect and trust and ensuring all voices are heard.
