Utilizes nursing experience and critical thinking to evaluate stroke patient care by conducting concurrent chart review through EMR, data abstraction and clinical rounds. Assists in the coordination of care and performance improvement with other service team members and accurately enters clinical data points into the databases. Actively participates in the achievement of stroke service goals to maintain appropriate stroke center designation.
Bachelor's Degree or equivalent experience Required
WORK EXPERIENCE REQUIREMENTS
Must have 3-4 years relevant healthcare experience; Prefer background in nursing and data management;
Current Colorado RN License
POSITION DUTIES (ESSENTIAL FUNCTIONS, INCLUDE % OF TIME)
Identifies stroke patient care issues/concerns and independently coordinates with other team members (attending physician staff, program manager, medical director, and nursing staff) to address in a timely fashion. Assures that developed protocols for Stroke are followed.
Maintains Stroke database, including data collection, entry and analysis. Develops and delivers reports as requested.
Participate in patient rounds on in-house stroke patients
Serves as a liaison between staff in the emergency department, IR, inpatient nursing units, ancillary departments, and physicians.
Reviews stroke alerts and assesses for accuracy.
May participate in community activities related to stroke care and injury prevention.
Actively participates in quality improvement initiatives and processes.
Assists with preparation for Joint Commission reviews and verifications.
Enhances professional growth and development through educational programs, current literature, in-service meetings and workshops to remain current with industry changes.
Adheres to professional code of ethics and to all accreditation and regulatory requirements.
Demonstrates effective time management and is proactive in prioritizing workload and meeting productivity requirements.
Adapts to rapidly changing technologies, policies, regulations, census and staffing through priority setting and flexibility.
