Summary Patient Resident Liaison
Performs and coordinates administrative communication functions to assure organized delivery of patient care services under the guidance of the medical staff team.
Delivers successful service to our internal and external customers by adhering to critical success standards: demonstrating accountability and commitment to quality and safety standards; taking ownership in the financial success of the hospital with appropriate resource use and management; providing exceptional customer service; and exerting effort to insure a positive work environment that demonstrates dedication to the hospital's mission, vision and values. Regular attendance, as outlined in the Attendance and Absenteeism Policy, is an essential function of this position.
Facilitates improving patient flow and work flow efficiency by organizing and prioritizing the processes related to capacity issues; including facilitating orders/studies and team communication.
Accurately maintains the organization of Patient Resident Liaison daily census by ensuring accuracy of: Patient Room, Name, Age DOB, Sex, Shift RN and Phone #, MR #, Shift Respiratory Therapist, Diagnosis (from MD's admission), Admit Date, Anticipated Discharge Date, Treatment Team, Attending Names and PCP Name. Also updates other appropriate systems (e.g. EPIC Karedex and PRL census) with Attending names, Resident names and phone #'s
Confirms communication to the patient's PCP regarding the patient's admission has occurred, and if not, updates PCP with appropriate information.
Contributes to successful customer service by providing accurate, timely, polite communications (verbal and written)
Facilitates health care providers work flow processes.
High School or equivalent
Completion of formal training course including medical terminology and order transcription, or on the job training
Basic Life Support certification through the American Heart Association with card in-hand before start date, and current through sixty days of hire
Previous clerical and data entry experience Associates or Bachelor's Degree in Business or Human Services related area
