Located in Fort Collins, Mountain Crest offers services to clients in northern Colorado, western Nebraska, and southern Wyoming. With accreditation by national and state agencies, Mountain Crest is a service of Poudre Valley Hospital, but operates in a modern stand-alone center on a beautiful campus with trees, lawns, a team-building ropes course, and other outdoor benefits for therapeutic use.

The Mountain Crest team works closely with each client to develop an individualized plan of care that includes family and, as needed, community members and professional services. The cornerstone of our philosophy is family involvement.
Free supervision by a licensed therapist, toward licensure in a multidisciplinary team with opportunities in group therapy, individual, case management and emergency room assessments of psych patients. Opportunities to learn DBT skills.
Summary Behavioral Health Counselor Provides assessment, planning, counseling, and education services for the behavioral health patient.
Crisis assessment skills needed for emergency room interventions, including determination of level of care needed. Case management skills desired to create supportive discharge planning to community. Collaboration with diverse clinical team to facilitate placement in a hospital setting.
Implements counseling case management for assigned patients. Provides group and individual therapy.
Utilizes appropriate teaching methods and/or referrals to ensure patient/family educational needs are met.
Assesses, collaborates and coordinates patient's discharge planning needs and readiness for discharge with other members of the care team.
Master's degree in Behavioral Health field.
State licensure in Behavioral Health, acceptable licensures: LSW, LPCC, LMFT, Lic. Psychologist, LCSW, LPC, RN, LAC, PhD. LSW and LPCC must be fully licensed within 2 years also accepted for North
1 year hospital experience
