Schedule: Part Time
The Case Manager is assigned to specific Hospital Units to facilitate the care plan for the inpatient/observation episode, proactively monitor clinical quality and facilitate timely discharge. Using a collaborative interdisciplinary approach, the Case Manager will confirm the plan of care and focus on anticipation, integration and coordination of clinical care and discharge planning services necessary to meet the needs of patients in order to optimize clinical outcomes and resource utilization.
Bachelors, RN with BSN preferred, Case Management Certification preferred
WORK EXPERIENCE REQUIREMENTS
3-5 years RN experience in an Acute Care hospital
Case manager experience in an Acute Care setting and/or Managed Care
RN/Case Management Certification (preferred)
Light Work - exert/lift up to 20 lbs. force occasionally, and/or up to 10 lbs. frequently.
