Summary - Pre-Liver Transplant Coordinator
Coordinates/optimizes health care team interventions across the continuum and assists in developing and meeting outcome management and system improvement goals. Provides the Nursing component to the case management triad and the management of patient/family care.
This position does not include utilization review, main responsibilities of position are liver disease management.
Coordinates care of patients across continuum in collaboration with triad and health care team.
Provides coordination of care consistent with accrediting and regulatory standards.
Performs select interventions/procedures, and interprets diagnostic tests within scope of nursing practice
Assists in designing patient/family education tools/systems and guides nurses in the use thereof. Assists in assuring resources are optimally used to facilitate patient care goals and appropriate cost and LOS.
Maintains timely and complete oral and written communication/ documentation and statistical and data reports per UCH standards Collaborates with triad/health care team in outcomes based practice in accordance with UCH strategic initiatives
