Job Summary Responsibilities include providing spiritual care to patients, families and staff experiencing various forms of spiritual/existential stress in a clinical environment as part of a multi-professional palliative care team.. Provides spiritual assessments and documentation of emotional and spiritual interventions of care in the patient's medical record. Communicates appropriate follow up information to the area Director & area CMO.
Responds to requests immediately by phone, assesses best response to spiritual care needs as part of the campus palliative care team, and either triages response by phone or responds in person to meet the spiritual care need. Debriefs and supports staff who are engaged in and managing emotionally charged situations and experiences. Utilizes empathic listening skills and training to evaluate patient and family spiritual needs. Communicates with sensitivity, clarity and compassion in telephone conversations.
Acts as a liaison and facilitator between patient, family, physician, hospital staff and community clergy during periods of crisis; e.g., lengthy and/or frequent hospital stays, difficult surgeries, impending death, death notification and trauma.
Serves as an educational resource to families and staff on a variety of topics related to spiritual care through consultation, and utilize resources efficiently by recommending or initiating referrals to appropriate health care team members. Initiate referrals to community clergy upon patient request.
Serves as available for network chaplain needs across all campuses as needed by executive sponsor of Network Palliative Care.
Documents Chaplain involvement with spiritual care in the patient's medical record and provide Chaplain Services Coordinator with appropriate follow up information.
Participate in continuing education opportunities to maintain professional standards and certification.
Actively participates in scheduled continuing education programs and chaplaincy team meetings
