Lead Behavioral Health Case Manager
Serves as the leader of the Behavioral Health Case Managers (to include the Behavioral Health Consult Case Manager and ED Behavioral Health Case Managers) under the direction of the Social Work Manager. Responsible for the assessment, planning, implementation, coordination, monitoring and evaluation of the patient's plan of care from admission to post discharge to include utilization review. He/She will utilize critical thinking skills and the principles of Case Management to coordinate and implement a discharge plan that meets the patient's needs and ensures a seamless, safe and efficient transition of care across the continuum.
