Performs patient observation, data collection related to patient care, administers medication, treatment and procedures according to the physiciansâ orders and resident care plan. Communicates patient findings to the supervisor. Assumes responsibility and accountability for facilitating, communicating and collaborating with the healthcare team to identify and meet the physical, emotional and spiritual needs of the patients. Oversees care provided by CNAs, Care Partners, QMAPs and/or other support staff. Promotes the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act. Demonstrates personal accountability for relationship-based care, organizational mission, and core values.
What You Bring/Job Requirements
In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:Â

LPN license in Colorado
BLS Certification through American Heart Association
One year of previous LPN experience, preferred
Requires knowledge in nursing and person-centered care concepts
Ability to demonstrate competency in the provision of individual care of patients based on the patientsâ assessed age and developmental needs.
Physical Requirements-Â Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly).
