To provide exceptional customer service while participating in the discharge planning process, assess, facilitate, plan and coordinate delivery of agency services. Collaborates with patients, families, physicians, hospital staff and internal customers to facilitate agency services to ensure a superb customer experience. Completes agency referrals, providing accurate demographic information and appropriate physician orders for care.
What You Bring/Job Requirements
In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:
Graduate of accredited school of nursing
Current Colorado RN license
At least one year of home care, hospice or home infusion experience
One to two years of discharge planning, care coordination or case management experience, preferred
Excellent customer service skills and a commitment to meet the needs of all customers and patients
Strong analytical, organizational, time management and problem solving skills requiredÂ
BLS ceritification through American Heart Association
Physical requirements-Â Sedentary work -(prolonged periods of sitting and exert up to 10lbs force occasionally)
