Sepsis Coordinator – Quality Management

HCA Houston Healthcare Medical Center is currently looking for a Sepsis Coordinator – Quality Management
Under the general supervision of the Quality Director, the Sepsis Coordinator is responsible for developing hospital wide educational curriculum for staff nurses and physicians focused on the early recognition of severe sepsis and activation of the Code Sepsis Team.; assessing the appropriate and early assessment of severe sepsis patient presenting the emergency department, the compliance with the sepsis bundle and issues related to the transfer of patient care to the ICU team; sepsis data collection and presentation to the hospital leadership and Medical Executive Committee.
ESSENTIAL FUNCTIONS OF THE POSITIONS
o Responsible for overall surviving sepsis program initiative and identifying areas for improvement in recognition, process of 'Code Sepsis/ Sepsis Alert' activation, compliance in completing the sepsis bundle and providing suggestion to improve outcomes.
o Evaluating hospital wide Code Sepsis protocol compliance and the completion of the Early Goal Directed Therapy Sepsis Bundle.
o Build team of sepsis champions including charge and unit nurse, key physician representing the Emergency Department, Intensivist Group, Hospitalists and Surgeons.
o Participating on ICU Multidisciplinary Rounds to follow up on 'Code Sepsis' patients and identify missed opportunities for 'Code Sepsis' activation.
o Conducting daily Sepsis Rounds during which unit patients and previous subjects of code sepsis are provided follow up and current length of stay, complications related to severe sepsis are addressed.
o Regularly meet with ICU, telemetry and ward nursing managers to provide follow up/ feedback in the surviving sepsis initiative and to provide suggestion for improved compliance in this initiative.
o Serve as a resource for nursing and physician staff members providing the education, tools and strategies in improving their understanding of severe sepsis syndrome and their compliance with the protocol.
· Sepsis Assessment and data collection:
o Responsible for hospital wide data collection including:
Total cases of severe sepsis, with and without shock
Sepsis Bundle Compliance within one hour of recognition and
activation of the sepsis team. Unit and physician compliance (ED
severe sepsis, severe sepsis with shock and severe sepsis without
· Quality Improvement:
o Leads initiative in reviewing the success of the Surviving Sepsis Initiative and providing suggestion for improvement in the process of recognition, activation and implementation of the sepsis protocol.
o Examines current CVC utilization and ensures resource access and availability all in an effort to improve the early process for early intervention and hemodynamic monitoring
