As a Nurse Care Coordinator, you are responsible for facilitating outpatient case management services for patients. The Nurse Care Coordinator generates effective personal plans for high risk participants, such as those experiencing one or more chronic medical conditions, enabling the participant with actionable goals leading to lifestyle management and health risk reduction.
The Nurse Care Coordinator also provides guidance for management of the at-risk population to mitigate increasing risk. Job roles and responsibilities emphasize a patient-centered, team-based approach to care and support each team being trained to the highest level of function by the state law.

Associate’s degree in Nursing is required.
Two (2) years clinical experience in an outpatient setting or Two (2) years of experience coaching individuals with complex and/or chronic conditions is required.
Prior experience in care coordination in a community setting or primary care practice is preferred.
