This position allows interns to work in unique areas of ambulatory care pharmacy: Atrium Outpatient Pharmacy, Pharmacy Admissions and Transitions of Care Services, Lowry Outpatient Pharmacy (providing coordination of Shingrix administration) and processing discharge prior authorizations as part of our Medication Access and Renewal Center (MARC). Based on individual performance, the successful intern will have the opportunity to work closely with pharmacists and nurses to provide clinical services in these areas. This internship does NOT involve inpatient pharmacy activities (IV Room, Clinical Pharmacy on Inpatient Units, etc.), and does not currently involve time in an ambulatory clinic.
During the school year--work a minimum of 12 hours/week throughout the school year and 32+ hours/week during summer and winter breaks, including rotating through one weekend shift approximately every 2 weekends.
Weekday shifts are typically in the afternoon/evening after your classes are done for the day (shifts end anywhere between 5:30 and 8:30pm).
**While interns CAN have another job in addition to this, we will not be able to arrange schedules around a secondary work location, and can only work around your school schedule.
Atrium Outpatient Pharmacy and Lowry Pharmacy (accounts for 70 % of shifts)
Appropriately fulfill medication orders and dispense prescriptions
Participate in transferring of prescriptions between pharmacies
Coordinate the fulfillment and bedside delivery of discharge prescriptions by collaborating with the patient, family members and their healthcare team
Conduct discharge medication counseling
Obtain prior authorization for discharge prescriptions
Coordinate scheduling of immunization administration for patients at Lowry Pharmacy
Prepare immunizations and document immunization
After P1 year, participate in administration of immunizations once certification has been obtained
Pharmacy Admissions and Transitions of Care Services (accounts for25% of shifts)
Perform medication reconciliation on newly admitted patients in order to reduce inpatient medication errors
Address discharge medication barriers
Assist with discharge medication reconciliation
Conduct discharge medication counseling or counseling about the patient's disease state
Provide a list of the patient's discharge medications to their community pharmacy
Follow up with the patient telephonically 2 days after discharge
Medication Access and Renewal Center (MARC) (accounts for <5% of shifts)
Complete prior authorizations for specialty medications by clinically reviewing provider office visit notes, pertinent labs and medication history
Identify appropriate financial assistance options so patients can afford their medications
Conduct periodic clinical assessments of patients taking specialty medications in order to assess medication adherence, occurrence of adverse effects and therapeutic effectiveness.
Assist pharmacists in writing appeal letters and/or identifying therapeutic alternatives for medications denied by insurance
Perform thorough chart reviews to evaluate the appropriateness of refill renewal requests
Enrolled in an accredited school of pharmacy and in good standing.
