Position Summary:This position supports the mission of Wesley Medical Center in optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. RN Case Manager facilitates the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, and admission status and resource management while ensuring the best possible outcomes within a fiscally responsible timeframe. RN Case Manager identifies potential barriers to throughput and quality outcomes and facilitates appropriate discharge plans in conjunction with social workers. RN Case Manager is responsible for working during varied shift schedules, actively participates in outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all. Daily responsibilities may include discharge planning activities which begin at the time of (or prior to) admission and are reassessed throughout the course of hospitalization.
