Social Worker III PRN
Position Summary: This position supports the mission of Presbyterian/St Luke’s Medical Center of optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. The Transplant Social Worker III PRN is responsible for assessing the psychological, social, cultural and/or financial needs that impact transplant patients. Has the ability to utilize social services, counseling and clinical knowledge in the assessment of these needs. Knowledgeable in ESRD/transplantation Medicare guidelines. Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patients and family needs with the efficacious and cost effective utilization of resources. Serves as a transplant social worker resource though out the facility. Knowledge of payer source implications on plan of care and resources to maximize patient’s successful outcome.

Licensure/Certification/Registration: Licensed Clinical Social Worker (LCSW) in the state of Colorado.
Education: Master of Social Work (MSW) from an accredited school of social work required.
Experience: 3-5 years recent clinical social work experience in transplantation. 5-7 years recent clinical social work in an acute setting.
Special Qualifications: Must have excellent written, verbal and interpersonal communication skills and able to work with highly diverse population. Must be knowledgeable of State and JCAHO standards. Must be knowledgeable of ESRD/transplantation Medicare guidelines. Must have ability to perform multiple tasks simultaneously and work in high stress environment. Must be able to apply critical thinking and formulate diagnostic impressions based upon review of systems, history, symptoms and current presentation. Must be able to provide crisis intervention and utilize system resources/referrals.
Degree of supervision required: Involves general guidance and direction by the Department Director. Employee will be expected to perform all job duties independently and in accordance with established departmental and hospital policies and procedures.
Ages of Patients Served: This position requires knowledge of growth and developmental needs of patients and families. Personnel in this position must recognize and respond to the behaviors of patients and families in each age category. The employee must be able to demonstrate the special skills required to care for patients and their families according to standards. (The skills and knowledge needed to provide such care may be gained through education, training or experience.)
