As an RN Director of Care Coordination, you need to know how to:
Promote and reinforce patient centered medical home concepts with patients and staff.
Work closely with providers, case management and clinical team to develop and deploy an operating model that helps ensure cost effective, appropriate utilization of health care resources and care management of high risk and rising risk patients.
Identify gaps in care for patients using eClinicalWorks, Azara and various other data resources including care gap reports from Rocky Mountain Health Plans, Medicaid and other commercial insurance carriers.
Assist with the development, revision, and coordination of the Plan of Care through collaboration with the multidisciplinary treatment team to meet the patientsâ™ needs effectively and efficiently.
Engage the resources of the payer when appropriate.
Develop new and enhanced department programs to improve the quality of health status and/or reduce unnecessary costs of health care services for the patient population we serve.
Follow-up with patients related to acute care visits such as Emergency Department/Convenient Care Utilization and Hospitalizations.
Provide leadership and oversight of care coordination functions including but not limited to care transitions, population health outcomes, and integration with providers and practice care teams.
Work in conjunction with Chief Medical Officer and Director of Quality to design and evaluate the program. Review program data and patient feedback to include making suggestions for and implementing process changes, improvements, program design and implementation of new initiatives.
Communicate with provider teams as needed related to chronic disease management and continuity of care issues to enhance quality of care for patients.
Oversee policy and procedure development and ensure meeting of delegation requirements. Be responsible for department compliance and compliance monitoring.
Recognize problems and responds, systematically gather information, sort through complex issues, seek input from others, address root cause of issues, make timely decisions, and can make difficult decisions; use consensus when possible, communicate decisions to others.
Set challenging and productive goals for the team.
Keep team accountable for actions.
Provide leadership and motivation; provide resources and support.
Use checkpoints and data to track progress; develop systems to measure results.
Manage process for identification and assignment of targeted populations to maximize effectiveness of care coordination services.

Graduate of an accredited school of nursing, required
Current Colorado RN license or compact RN license with the ability to obtain Colorado RN license, required
Minimum of three (3) years of healthcare management experience, required
Minimum of two (2) years of experience in an ambulatory care or family practice clinic, community health center, a public health setting or a healthcare related environment, required
Bachelorâ™s Degree in Nursing (BSN), preferred
