This is a PRN (as needed) position to cover when Full Time staff are out.
Here is some of whatyou can expect from being an RN Case Manager with us:
Youwill coordinate services through an interdisciplinary process, which provides aclinical, financial, and psycho-social approach through the continuum of care
Youwill balance individually identified patient and family needs with theefficacious and cost effective utilization of resources
Youwill assess patients to determine appropriateness of admission, continuedhospitalization, clinical resource consumption, progressive care management, aswell as appropriate level of care
Youwill participate in discharge planning at the time of (or prior to) admission,and reassess throughout the course of hospitalization
Youwill support collaboration and communication among the patient/family andentire health care team regarding the patient's plan of care and continuedneeds post-discharge and facilities those services required
Youwill perform telephonic clinical reviews with claims insurance personnel tofacilitate reimbursement for the delivery of patient care services
Youwill identify quality and/or risk issues and reports to the appropriateindividual/department
Qualificationsyou will need for this role:
Currentlicensure in the State of Colorado as a Registered Nurse, or current activemultistate nursing licensure
BLScertification through AHA or ARC
CaseManagement or CPHQ certification strongly preferred
Graduateof an accredited school of nursing; BSN preferred
Skill inassessing planning and managing patient care delivery as acquired thorough aminimum of 2 years of clinical nursing experience in the acute care setting
Priorutilization/quality/discharge planning/and/or case management experience highlypreferred
Solidclinical knowledge base in medical/surgical nursing
Abilityto perform effectively in team situations
Abilityto assess information and apply critical thinking processes to determine courseof action and relay this information to the interdisciplinary team
Excellenthuman relations and oral/written communication skills
Aboveaverage organizational/prioritization skills
