Denver Health is recruiting a LCSW/LPC to join our team! Job Summary The LCSW/LPC provides therapeutic services for patients receiving mental health treatment. Patient and family services may include clinical assessment and diagnostic skills, brief therapeutic interventions, care plan development, case management, referral to other medical services and discharge planning. Clinician utilizes an evidence-based framework with a multicultural and trauma-informed lens to support patients in their path to recovery. Clinician functions as an integral member of the psychiatric consult liaison team.This is an interdisciplinary team fulfilling patient care across the outpatient and inpatient medical settings.Services uphold Colorado Office of Behavioral Health (OBH) and Licensure Standards. Clinician is actively involved in quality improvement, data monitoring, and in research projects.
Responsibilities Responsible for direct patient care. All documentation must clearly address the patient's individual life areas, in need of clinical support, to achieve positive clinical outcomes. Facilitates both educational and therapeutic groups as well as individual sessions and fundamental case management. 1. Functions as a social worker on the multidisciplinary inpatient psychiatry treatment team, providing psychiatric treatment, coordination of care and discharge plans as it pertains to the behavioral health needs of the client and family.2. Co-facilitates or facilitates family sessions throughout the duration of admission that relate to the collection of collateral information, diagnosis, prognosis, family communication, increased coping and psychiatric safety planning.3. Manages the scheduling of family meetings to align with family availability and to prevent double-booking across provider disciplines. 4. Conducts comprehensive biopsychosocial assessments that are thorough, individualized, objective and complete. 5. Participates in the implementation and execution of treatment plans, including daily multidisciplinary rounds and weekly treatment plan reviews as needed.6. Applies knowledge of individual, group and family functioning to ensure appropriateness of treatment methods to assist client in achieving treatment goals. Is aware of and able to articulate the professional use of self in the relationship to maximize client's adjustment, enhance strengths and promote resolution of identified problems. Demonstrates ability to develop effective therapeutic interventions that are socio-culturally sensitive.7. Utilizes clinical skills to provide treatment recommendations to clients and families. Coordinates follow-up treatment referrals to various levels of care including short-term residential treatment, long-term residential treatment, partial hospital programs, intensive outpatient programs, in-home therapy, day treatment, and outpatient therapy/psychiatry as indicated.8. Other duties as assigned.
Knowledge, Skills & Abilities 1. Knowledge of crisis intervention and brief and long term therapy processes. 2. Knowledge of patient populations' cultural, religious, ethnic, and social systems in interactions, care planning and education. 3. Knowledge of medical terms, abbreviations, diagnosis, treatment and prognosis and demonstrates this knowledge verbally and in writing. 4. Skill in navigating and documenting within a medical record system.5. Skill in providing care appropriate to the age of the patients served within the unit. 6. Skill in establishing a treatment alliance and engaging the client system in goal setting and prioritizing. 7. Ability to manage the professional relationship including termination. 8. Ability to respond to trauma or psychiatric emergency.
Education * Master's Degree (Required) 1. Master's Degree in Social Work from a CSWE accredited college or university, required.2. Two years of full-time post Master's Degree social work practice under the supervision of a licensed or equivalently qualified social worker or other DORA approved professional practitioner, required.
Experience
Credentials * Basic Life Support (BLS/CPR) from American Heart Association (Required) * Licensed Professional Counselor* Licensed as a LPC, LCSW, LMFT or licensed Psychologist. CAC II or III preferred. Location Shift Work Type All job applicants for safety-sensitive positions must pass a pre-employment drug test, once a conditional offer of employment has been made.
