Denver Health's Ambulatory Care Services provides outpatient Clinical Social Work services, which are available to help patients and families cope with the biopsychosocial stressors often associated with illness and/or injury. This service helps to identify those patients with complex medical problems, who are socially disadvantaged and present with multiple barriers to care. Successful integration of the Social Worker within ACS helps to improve the quality of care for the patient, assists the outpatient clinic with management of psychosocial issues/challenges and helps to promote an emphasis on patient empowerment and self-management goals.
Under minimal supervision, plans, organizes and implements social work treatment for patients and families. Collaborates with the orthopedic surgeon's team and inpatient care management to prepare patients for a successful perioperative experience. Assesses the biopsychosocial needs of patients and families and creates an individual plan of care, as it relates to the patient's medical care. Provides clinical interventions that help meet identified needs by utilizing personal, family and community resources. Provides short-term crisis intervention, supportive counseling, case consultation and care coordination services.
1.Conducts comprehensive biopsychosocial assessments that are thorough, individualized, objective and complete.Provides intervention appropriate to the presenting problem.
2.Utilizes key case management components including assessment, care planning, coordination of care, monitoring, follow-up and documentation.
3.Evaluates emotional components, impact of health issues and assists patient to identify strengths, dysfunctional factors and to establish detailed and coordinated care plans.
4.Provides care coordination throughout the continuum of care from case referral to case closure.Utilize/delegate activities to the Care Navigator as appropriate, for more administrative or time intensive tasks.
5.Applies knowledge of individual, group and family functioning to ensure appropriateness of treatment methods to assist patient in achieving care plan goals.
6.Is aware of and able to articulate the professional use of self in the relationship to maximize the patient's adjustment, enhance strengths and promote resolution of identified problems.
7.Demonstrates the ability to develop effective therapeutic interventions that are socio-culturally sensitive.
8.Routinely consults, negotiates, and coordinates with internal and external resources to assure collaborative efforts to maximize patient outcomes.
9.Demonstrates leadership in facilitating multi-disciplinary communications and patient care meetings (i.e. case conferences) and utilizes information to assess and reassess care needs.
10.Demonstrates and educates staff as to the scope of function of the Social Work role in the health care team.
11.Ensures patient involvement in care planning and goal setting with informed consent documented.
12.Complies with applicable laws and educates other staff as to legal issues related to Social Work practice and/or issues specific to the clinic Care Team.
13.Prepares written and electronic documentation as required.
14.Charting is thorough, timely and content reflects established qualitative standards.
15.Demonstrates knowledge and applicability of the principles of growth and development over the life span, as well as demonstrating the ability to assess the patient's status and interpreting appropriate cultural information of the patient(s) to whom care/service is being delivered/provided.
16.Demonstrates knowledge of major concepts principles and theories of normal and abnormal human development, and its application to specific age groups.

Master's Degree in Social Work from a CSWE accredited college or university required.
Two years of full time post Master's Degree social work practice under the supervision of a licensed or equivalently qualified social worker or other DORA approved professional practitioner required. 3. Three years of directly related experience required.
