The is Advanced Practice Clinical Coordinator an individual who demonstrates the knowledge and skills necessary to lead stroke services at SRMC in providing specialized care to the stroke patient population. The coordinator is accountable for oversight of the clinical stroke program to assure that all patients receive comprehensive and timely treatment interventions that are consistent with Sky Ridge Medical Center’s defined Clinical Practice Guidelines. Oversight of the patient achieved through collaboration with the health care team to provide patient education, participating in multidisciplinary rounding, and assists with facilitating the bedside clinical care of Stroke/TIA and other complex cerebrovascular patients to assure that all patients receive comprehensive and timely treatment and interventions. Quality assessment of patient care to include assisting with design and development of PI indicators, active participation in outcome review, and initial screening for peer review issues. All stroke registry data and PI activities will comply with Joint Commission, state and national guidelines. Provides ongoing statistical reports and assists with developing specialized reports as required or assigned. The coordinator demonstrates an understanding of program development, implementation, evaluation and marketing. Serves as a liaison between the Stroke/Neuroscience service and related departments. The Advanced Practice Clinical Coordinator provides formal education to staff and the community related to cerebrovascular-specific topics, and acts as a resource for staff, EMS partners, the community and physicians, to support Stroke/cerebrovascular program goals. The coordinator organizes, coordinates, and facilitates meetings for the Stroke Services Program and performs delegated management duties assigned by the Director of CV services, which are consistent with her/his, scope of practice and competence. The Scope and complexity of the Advanced Practice Clinical Coordinator position will vary based on program growth and variety of services, number of medical staff and medical staff services involved the activity level of admissions, discharges and transfers per day, and the education resources available.

Current licensure in the State of Colorado as a Registered Nurse, or current active multistate Registered Nurse license
Graduate of an Accredited School of Nursing. Completion of an accredited program for advanced practice nurses.
