As a Care Manager RN you need to know how to:
Collaborate with the health care team to develop a plan of care by participating in patient care
Demonstrate knowledge of the principles of growth and development over the life span and possesses the ability to differentiate the care that is required based on the patients’ current place on the life growth and development flow
Facilitate the safe care of patients including transfer of care to alternate level of care settings, assuring appropriate exchange of information among all caregivers
Identifies patients requiring care management and takes the lead as care manager for those requiring intervention, especially with socially complex cases.
Utilize motivational interviewing skills to support health behavior change in Bruner Family Medicine patients
Provide education to other departments/clinics regarding care management services as requested
Participates in co-visits with providers using a multidisciplinary approach as needed.
Serve as support to Bruner Family Medicine for population management, patient self-management, along with support and tracking and intervening with high utilizers
Coordinate patient centered care to reduce fragmentation of services
Interview patients and families to assess current strategies for self-care and available support systems to manage their health
Serve as a support to Bruner Family Medicine in the area of social services, Medicaid applications, disability eligibility, ETOH resources and homeless resources
Use SW knowledge and skills to assure the health care team understands and supports patient self-determination when appropriate
Partner with community resources and community referrals sources as well as build new relationship with the direct community for patient care needs
We hire people, not resumes. But we also expect excellence, which is why we require:
1 year Care Manager experience in Primary Care setting
Current BLS certification
Graduate from Accredited Nursing Program
Current Nursing License in the State of Colorado
2 years Registered Nurse experience in Family Medicine or Internal Medicine office practice
Experience working with, and sensitivity toward, special populations, including those with limited income, and limited education, and with non-English speaking individuals
Working knowledge of Microsoft Office programs
Full understanding and working knowledge of EPIC computer systems
Experience with PC software applications in word processing, Word, and Excel
