As a clinical employee that serves the needs of young, middle and older adults in Behavioral Health Services, the position of licensed therapist/care coordinator is responsible for providing psychotherapy and care management services. The position requires the ability to manage treatment, and continuity of care from admission through discharge and coordination of insurance benefits. The position necessitates leadership skills with the ability to direct and evaluate all aspects of patient care, and to assure that they are performed in a timely, effective and efficient manner. Working knowledge of managed care, its terminology, structure and process is preferred. Excellent organizational skills and the ability to work and communicate well with others are critical aspects of this position.
Education or Formal Training
Academic background in the area of social work, nursing, counseling, psychology, or related field with a minimum of master’s degree as required for licensure eligibility.
Special Qualifications (Licensure, Registration, etc.)
Must be licensed LPC/LCSW or eligible for licensure.
Colorado LPC or LCSW Licensure required within one year of hire.
Knowledge, Skill and Ability
Patient-care model of time-effective therapy.
Ability to assess and evaluate a wide range of psychosocial impairments within a patient population 18 through the continuum of life.
Working knowledge of current state and federal laws relating to mental health (27-10 and Standards and Guidelines). Working knowledge of Joint Commission rules and regulations.
Working knowledge of Medicaid/Medicare regulations.
Knowledge of healthcare systems and reimbursement systems.
Ability to perform thorough social and mental assessments.
Skill in the coordination and attainment of healthcare services.
Excellent verbal and written communications ability.
Computer literacy and ability to use word processing.
Demonstrated knowledge of DSM-V and its use.
Demonstrated ability to deal tactfully with all clients, third-party payors, attorneys, all hospital staff and physicians.
Knowledge of and experience with addiction, recovery and mental health issues.
Familiarity with chemical dependency issues and 12 step programs is required.
2-3 years’ experience in a psychiatric setting inpatient and/or outpatient preferred.
Recovery experience preferred.
Experience with a multi-disciplinary team approach.

Accomplish tasks while working under pressure of consistent deadlines.

Employee must be able to consistently support, push, pull and/or lift up to 10 pounds; occasionally support, push, pull and/or lift up to 30 pounds and occasionally support, push pull and/or transfer up to 50 pounds (for example, moving a patient from a wheelchair to a bed).
May serve the HIV+ patient population
Ability to maintain internal contacts with physicians and employees, ability to act as resource and motivate these individuals.
Interdepartmental relationships throughout the hospital.
Ability to communicate effectively with public and self-help organizations regarding available resources at Boulder Community Hospital and the community at large.
Ability to coordinate services with medical staff, treatment programs and third-party payors.
Professional relationship with patients and families.
Ability to communicate effectively with community resources related to disposition planning.
Provide clinical supervision for interns per Program Manager’s request.
Provide clinical supervision for similar license track staff, per Program Manager’s request.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Demonstrate ENCORE behavior and promote ENCORE philosophy and activities with all BHS staff.
Support BHS mission and provide leadership for open, direct communication and conflict resolution.
Conduct all patient-care activities with respect for patient’s privacy and right to confidentiality and autonomy.
Use discretion in relaying administrative concerns, decisions or discussion with other staff.
Provide in-services for staff, when needed, per specialty licensure.
Provide coverage, per Program Manager’s request, for vacations, leaves of absence, etc.
Maintain certification and licensure per state regulations and as required by discipline and/or job qualifications.
Assume clinical leadership role for program team regarding referrals, orientations, billing and UR concerns, and adherence to appropriate staffing patterns.
May perform other duties as delegated by Program Manager.
Oversees quality indicators at the program level and reports data quarterly to Program Manager.
Performs chart reviews per Program Manager’s request.
Demonstrates responsibility toward budget concerns and regularly communicates variances to the Program Manager.
Assist in admission and orientation of patients.
Review initial assessment(s) and assist in completion.
Conduct patient assessment evaluation and communication to the attending M.D. and treatment team.
Provide leadership in completion of the Master Treatment Plan and ongoing treatment goals.
Develop, coordinate and complete discharge planning.
Facilitate patient access to needed resources and aid in the development of alternatives as necessary to meet identified needs.
