Hospital Admissions Coordinator
ESSENTIAL FUNCTIONS: Responsible to understand "swing" post-acute requirements including conditions of participation that are unique to Critical Access Hospitals. Review and assess referrals from other facilities for medical, psychosocial, functional and financial appropriateness and preauthorization. Respond to referrals in a rapid, concise, efficient manner. Ideally this would occur during daily visits to local hospitals. Do on-site visits for patients at local referring facilities, educating the patients on what to expect while at Family Health West.
