Position qualifications: Bachelor's degree required, preferably in human services or related field; Two (2) years previous experience in human services related field preferred. Current RN/LPN Licensure in lieu of Bachelor' degree is acceptable.
Job purpose: It is Mind Springs, Inc. policy that a Medical Case Manager is assigned to each outpatient provider. The Medical Case Manager works closely with the psychiatric provider and other treatment team members, as necessary, to facilitate the provision of psychiatric services to Mind Springs clients. Under close supervision and oversight, develops and monitors treatment plans and progress, documents significant interactions, and maintains clinical charts and other reporting data.

Knowledge, Skill & Ability (include materials and equipment directly used): * Ability to produce written documentation of clinical work in a manner readily understood by other service providers. * Ability to summarize service themes from a variety of client contacts and communicate suggested improvements for service delivery. * Ability to demonstrate appropriate clinical judgment when assessing the level of risk and determining resolution of client emergencies, resulting in a safe outcome. * Working knowledge of Windows based computer applications to include Microsoft Word, Microsoft Excel, Microsoft Outlook, and Internet access. * Ability to operate standard office equipment including computer keyboard, calculator, copy machine, fax and multi-line telephone. * Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers. * Ability to organize and prioritize workload in a sometimes hectic environment with frequent interruptions.
Working Conditions/Physical Demands: Employee must occasionally lift and/or move up to 25 pounds. Employee is frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods of time at a computer. Employee must be able to travel between Mind Springs, Inc. sites or within the community. May be exposed to stressful situations that involve enraged, distraught and/or intoxicated individuals with possible mental disorders. Possible potential for exposure to communicable disease.
