Your day.
As a Home Health SLP you need to know how to:
Provide service to patients who have speech and language problems.
Participate in the development of speech therapy plans of care for patients.
Make visits to the patients for initial and ongoing assessments to identify the patientâ™s level of functioning and ability to communicated, including OASIS Assessments at appropriate time points.
Communicates with the physician before start of care and as needed.
Implement the plans of care.
Provide education and instruction to team members, patients and families.
Document care and submit paperwork in an accurate and timely manner to update and maintain the patient medical records.
Your experience.
We hire people, not resumes. But we also expect excellence, which is why we require:
Masterâ™s degree in Speech Therapy from an accredited institution, required. Licensed and in good standing in state in which he/she will practice, required.
Ability to demonstrate problem solving and decision making skills, required.
Certification in Speech Therapy by the American Speech and Hearing Association, required.
Knowledge of speech therapy principles, concepts and methodologies appropriate for terminally ill patients, required.
Excellent communication skills, both verbal and written, required.
Ability to develop and maintain rapport with the patient and families, required.
Minimum of two (2) yearsâ™ work experience as a Speech Therapist in a multidisciplinary setting, required.
