Knowledge and skills related to diabetes and risk-factor management are required.  Assumes responsibility and accountability for facilitating, communicating, and collaborating with both the healthcare team, and the patient/family to identify and meet the physical, emotional and spiritual needs of the patient. Promotes the optimal health, well being and safety of the patient through use of established processes and in accordance with patient care standards, guidelines and the State Nurse Practice Act (if applicable) and ADA. Demonstrates personal accountability for relationship-based care, organizational mission, and core values. The Diabetic Educator will work cooperatively with staff, residents and faculty (if applicable) to identify and provide educational services and to establish a practical, quality POC for diabetic patients while acting as leader, innovator and change agent in the pursuit of improved quality of patient careWhat You Bring/Job Requirements:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following: * Bachelors degree, required* Two year experience in diabetes education, required* Must possess practical application skills and ability to motivate clients to change* Experience with counseling patients who have chronic disease* Demonstrable experience with staff education related to diabetes mellitus.* Certified Diabetes Educator, required* Registered Nurse in the state of Colorado, required.Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantlyYour Connected Community:When people and places align, incredible things happen.
