This hybrid grant-funded role (through 12/31/2026) focuses on primary care initiatives including: Diabetes management Medication management Transitions of care Telehealth expansion Collaboration includes TennCare and Employee Health to improve patient care and pharmacy services.
Participates in all activities related to medication therapy management in area of specialty. Serves as an education resource for pharmacists, residents, students, and other health care professionals. Responsible for drug policy development and expert recommendations in area of specialty.
Pharm.D. degree from ACPE accredited College of Pharmacy, ASHP accredited residency required, and Tennessee licensure.
