About the opportunity:
The Concierge Nurse is a key member of the Concierge Medicine Program at Vail Health, providing clinical support to the concierge physician and advanced practice provider (APP). This role is designed to ensure high-quality, personalized patient care, seamless operational efficiency, and optimal patient satisfaction within this high-touch, patient-centered healthcare model. The Concierge Nurse will assist with clinical duties, patient care coordination, and operational tasks, contributing to the successful development and sustainability of the program.
What you will do:
Serve as a primary point of contact for concierge patients, managing appointment scheduling, follow-up reminders, and care transitions.
Ensure patients receive timely communication about treatment plans, lifestyle modifications, and changes in care.
Coordinate referrals to specialists and follow up with patients? post-consultation or procedure.
Work alongside the concierge physician and APP to conduct patient assessments, including physical exams, diagnostic procedures, and wellness checks.
Monitor patient conditions, track progress, and document health status in patient records.
Provide patient education regarding health conditions, treatment options, and wellness strategies.
Organize and maintain accurate patient medical records in compliance with HIPAA regulations.
Oversee inventory management of medical supplies and assist with ordering necessary equipment and medications.
Assist the concierge physician and APP with documentation, including progress notes, care plans, and follow-up documentation.
Monitor patient flow through the program to ensure a seamless experience for patients and providers.
Serve as a trusted liaison between patients and providers, ensuring a high-touch, personalized experience.

What you will need:
Experience:
2 years experience with applicable patient care and administrative duties preferred.
Basic knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations.
License(s):
Licensed as a Registered Nurse in the state of Colorado or from a valid compact state required.
Certification(s):
Basic Life Support (BLS) required
Computer / Typing:
Must possess, or be able to obtain within 90 days, the computers skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
Must have working knowledge of the English language, including reading, writing, and speaking English.
Education:
Minimum of Associate Nursing Degree for positions hired after 07/01/2018.
