About the Role

The Congressional Medal of Honor Society is seeking a mission-driven Associate Healthcare Coordinator to support the organization’s commitment to providing exceptional healthcare, medical, and wellness support to Medal of Honor Recipients, their spouses, and families. This position plays a vital role in expanding the reach and depth of the Society’s healthcare model, ensuring that each Recipient and family member has access to the highest standard of care and coordinated services. The ideal candidate will have a clinical background, experience in case management, and a passion for service and advocacy.

Key Responsibilities

• Regular and problem focused outreach to recipients, wives, and families to assess healthcare, medical, and wellness needs
•  Shares in the design, refinement, and administration of all service programs under HonorCare umbrella
• Assist in information and data gathering, record keeping, and outcome reporting
• Provides creative problem solving and constructs care plans to meet individual needs
• Sets follow ups to monitor care plan progress, recognizing need to pivot for changes or additional challenges
• Serves as liaison with medical providers including physicians, hospitals, case managers, health care plan providers
• Works collaboratively to build effective, long term relationships with key stakeholders including the VA, Tricare contractors, and private insurance to expedite problem solving and benefit tracking.
•  Educates on matters of health and wellness in a variety of areas
• Reviews and interprets information on benefits and develops pathway to ensure maximization
• Research of community resources to determine best option in areas of long term care, rehabilitation facilities, or home health care to assist decision making
• Builds communication and engagement with widows and next of kin
• Assists with risk management and planning for medical and safety needs at Recipient attended events

• Qualifications & Experience

  • Minimum of 5 years of clinical experience in a healthcare setting.
  • Bachelor's degree required.
  • Experience in case management and care coordination is essential.
  • Demonstrated ability to manage confidential information with professionalism and discretion.
  • Active, unrestricted clinical credentials (RN, LPN, or PA) required.
  • Excellent interpersonal, organizational, and communication skills.
  • Must be self-directed, compassionate, and capable of managing complex health situations with sensitivity and efficiency.
  • Experience navigating healthcare in DoD and Veterans Health Care Systems.
  • Understanding / knowledge of the Medal of Honor is not required but highly preferred.
