The Opportunity:

Shift: Full Time, Monday through Friday, day shift
Job Duties:
- Evaluate patients' psychosocial status and readiness for transplantation
- Assist patients in understanding and navigating the transplant process
- Facilitate access to community resources and support services
- Advocate for patients' needs within the healthcare system
- Develop and implement psychosocial interventions to enhance patients' quality of life and success of transplant
- Participate in multidisciplinary transplant team meetings
- Document patient interactions and maintain accurate records
- Provide crisis intervention as needed
- Assist with end-of-life planning and decision-making processes
- Communicates and collaborates regularly with multidisciplinary team
- Assists in development of the discharge plan
- Makes post-acute referrals and follow through to completion, including placements and transportation
- Mitigates discharge delays through timely interventions.
Transplant Social Worker Qualifications & Skills
- Experience working in a healthcare or medical setting
- Strong communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team
- Proficiency in crisis intervention and conflict resolution
- Understanding of financial assistance programs available to patients
- Competence in cultural sensitivity and inclusivity
- Experience in patient education and advocacy
- LMSW or equivalent clinical licensure
- Master's degree in social work (MSW)
- Minimum of 2-3 years of experience in a healthcare setting
- Strong knowledge of psychosocial assessment and interventions
- Excellent organizational and time-management skills
- Ability to handle sensitive and confidential information
- Empathy and compassion for patients and families
- Proficiency in medical terminology and healthcare systems
- Strong problem-solving and decision-making abilities
- Commitment to ongoing professional development and education
