UCLA Health is seeking a dedicated Senior Care Physician to join our innovative Senior Care Model clinic in Santa Monica. This role is designed to deliver high-value, patient-centered primary care for adults aged 65 and older. Built on the Age-Friendly Health System’s "4Ms" framework-What Matters, Medication, Mentation, and Mobility-this clinic will provide longitudinal primary and acute care services tailored to the needs of the aging population.
Provide comprehensive primary care for a smaller patient panel to enhance patient access and personalized care.
Enjoy sharing quality visit times with your patients with longer visits
Work in a collaborative, interdisciplinary team including Clinical Advisor RNs, Comprehensive Care Coordinators, and Licensed Clinical Social Workers to support patient care.
Dedicate 32 hours per week to direct patient care, with additional time allocated for administration and chronic care management.
Develop professionally through specialized education focused on chronic care management, value-based care, and population health strategies.
Collaborate with the Senior Care Model Medical Director to drive quality improvement initiatives and evaluate new care delivery models.
Receive a competitive salary plus quality-based incentives linked to panel size, acuity, and performance on key quality metrics.
The University of California, Los Angeles is required to provide an estimate of the salary range for this role. This range takes into account the wide range of factors that are considered in making compensation decisions. Salary offers are determined based on candidate qualifications and experience. The target base salary range for this position is 270,000.00-300,000.00 annual base salary. Productivity models, Quality Incentives, and additional income channels may supplement base salary for a total compensation of 450,000 annually.
Qualifications
Board certified/eligible in Internal Medicine or Family Medicine, required.
Board certified/eligible in Geriatric Medicine, preferred.
California licensed or licensed-eligible, required.
Passion for delivering patient-centered geriatric care and improving health outcomes for older adults.
Ideal candidates will be comfortable with telehealth appointments, as needed.
