The role you’ll contribute:
The Lead Nurse Navigator, in collaboration with the other Nurse Navigators, is responsible for the overall quality and functioning of the specialty or site-specific navigation program. They support clinical standards and initiatives set by the department and serve as a clinical resource, coach and liaison to other team members, with the goal of assuring exceptional patient care is provided. They consistently and appropriately problem solve day-to-day issues and assist with smooth patient flow. The Lead Nurse Navigator effectively communicates with staff, physicians, and leadership. They provide teaching to other team members. They support and assist with data collection for process improvements and work closely with the Quality team to ensure accurate data collection and reporting. The lead navigator works with management to assure all Centers for Medicare and Medicaid Services requirements applicable to their role are met. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
The value you’ll bring to the team:
· Maintains own patient assignments as a nurse navigator to navigate an identified patient from the diagnostic phase through the treatment phase, and survivorship
· ensuring appropriate/timely medical/supportive services
· Serves as the primary liaison to physicians’ offices, collaborating with the office staff regarding the necessity of appropriate orders, complete medical records,
· Consistently and appropriately problem solves day-to-day issues. Serves as a clinical and practice resource to other nurse navigators and coordinators
Qualifications
The expertise and experiences you’ll need to succeed:
· Bachelor's of Nursing
· 4 years of experience
· Kansas RN License
