SUMMARY:
The Staff Provider will examine and treat common acute illnesses, injuries, accidents and other injuries/illnesses; performs most medical services routinely handled within the scope of the advanced practice registered nurse’s practice and which the Staff Provider is competent to perform; and performs selected laboratory testing, refers to specialists as needed, writes prescriptions and performs office procedures within the scope of expertise protocols and available equipment. The Staff Provider is also responsible for delivering behavioral therapy and psychiatric care medication management within Gracelight’s integrated primary care setting, which includes coordinating and developing treatment plans to address patients' mental health, substance abuse, trauma, and psychosocial needs.
The provider will participate in primary care after-hour call rotations by telephone only. After-hour call schedule is dependent on staffing set at approximately 1 week every 4-5 weeks for now.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

Develops reasonable plan including treatment, referrals and use of drugs within scope of practice.

Provides and consistently documents relevant education.
Responds to patient special needs and appropriate requests including PAP drugs.
Works appropriate hours to minimize patient wait time as well as help to relieve overload situations/help other providers.
Acts and communicates in a professional manner to reduce friction with patients, providers, and staff.
Attends appropriate meetings as requested.
Maintains professional appearance in accordance with Gracelight Community Health’s policies.
Participates in peer review and orientation activities as requested.
Participates in committees when requested.
Cooperates with use of electronic medical records as requested.
Uses discretion and judgment in handling sensitive or confidential information.
Complies with organizational policies and procedures.
Must be willing and able to work at all locations as needed to meet patient care needs.
Performs all other duties as assigned.
Cooperates in call panels as scheduled.
QUALIFICATIONS:

EDUCATION/EXPERIENCE:
Advanced Practice in Registered Nurse.
Bachelor’s Degree from a 4-year college or university.
Current, valid Nurse Practitioner license in the State of California’s Board of Registered Nursing.
Submitted application as Nurse Practitioner Practicing Without Standardized Procedures in a Group Setting (103 NP) or submission of application within six months of employment.
Experience in psychiatric care medication management, preferably in a primary care setting.
Experience managing trauma informed care
Excellent written and verbal communication skills, utilizing, tact, active listening, empathy, and compassion. Able to manage conflict in a direct and professional manner.
Experience working with adults, children, and families from diverse social, cultural, educational, and economic backgrounds.
PREFERRED:
Previous ambulatory clinical experience above and beyond clinical training.
Previous FQHC experience.
Prior experience with Patient Centered Medical Home model of care delivery.
Working knowledge of medical Spanish.
LANGUAGE SKILLS:
Ability to read and interpret documents, such as policies and procedures, benefits information, benefit surveys, board minutes, routine mail, simple contracts, and procedure manuals. Ability to compose routine reports and correspondence. Ability to speak effectively with employees, visitors and management. Bilingual Medical Spanish preferred.
MATHEMATICAL SKILLS:
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, and the ability to apply concepts of basic algebra and geometry.
