Job Title: Care Management-Consultant (Remote Position)
Location: Riley Hospital
Shift: Monday - Friday, 8:00 AM - 4:30 PM
Position Overview:
The Care Management Coordinator supports the Integrated Care Management team by providing education, discharge planning, and post-acute vendor follow-up to enhance patient outcomes and reduce readmissions. This role involves collaborating with stakeholders, verifying admission information, and ensuring timely documentation in line with CMS guidelines. The coordinator acts as a key resource in verifying transfer authorizations, especially for NICU patients, and supports continuous quality improvement efforts within the IU Health Collaborative.
Key Responsibilities:
Consult with all necessary stakeholders to train and educate on various aspects of the Integrated Care Management process to achieve top-quality results.
Support discharge planning activities, including referral placement to home health, extended care facilities, and other post-acute services.
Verify patient admission information and ensure all documentation is completed timely to meet CMS guidelines.
Assist in decreasing hospital readmissions by ensuring primary care physician appointments are scheduled post-discharge.
Monitor and support post-acute vendor outcomes, ensuring timely follow-up on patient concerns and tracking quality data in alignment with the IU Health Collaborative.
Handle hospital transfer authorizations, particularly for NICU patients, ensuring proper documentation and approval.
Participate in staff meetings, including one-on-one staff meetings every other Monday (based on huddle day) and monthly Tuesday staff meetings at Riley Hospital.
Perform admission calls to verify important patient information and facilitate seamless transitions of care.
Serve as a mentor to team members, demonstrating a deep understanding of hospital systems and a proactive, go-getter attitude.
Manage multiple tasks efficiently, balancing system navigation, documentation, and communication while maintaining attention to detail.
Qualifications & Requirements:
Experience: 0-3 years of related healthcare or care coordination experience preferred.
Education: Bachelor's Degree or equivalent years of experience required.
Skills & Knowledge:
Strong communication skills, able to effectively collaborate with diverse stakeholders.
Advanced proficiency in Microsoft Excel, Word, and PowerPoint.
Excellent organizational and project management skills.
Strong analytical and problem-solving abilities.
Ability to multitask effectively, handling competing priorities in a fast-paced environment.
Experience working with different healthcare systems and navigating hospital workflows.
Additional Details:
Location: Remote, with required in-house presence 3 days a month at Riley Hospital (Tuesdays for staff meetings, every other Monday for huddle day).
